You can download a paper version of our Registration Form here (PDF format), complete all the sections as requested, and send a scan / photograph or copy of the completed document by email to mypet@westbarvets.uk.
Completed paper registration forms can also be handed into reception or posted to the practice.

Registering your pet on line or on paper really helps our team to assist owners with sick pets more quickly.
However if you are already a client of the practice or you have been with us in the past you can call us during office hours to register a new or additional pet.
